Program Terms & Conditions
Eligibility: Open to existing AX Health Insurance customers and general public. Referrers must be 18 years or older and provide valid contact information.
Qualifying Referrals: Referred individuals must enroll in and maintain a qualifying health insurance policy for at least 60 days from the effective date.
Reward Payment: Referral rewards will be paid within 30 days after the referred policy has been active for 60 days. Payment method will be determined during program enrollment.
Limitations: Self-referrals are not permitted. Referrals must be new customers who have not worked with AX Health Insurance in the past 12 months.
Program Changes: AX Health Insurance reserves the right to modify or terminate the referral program at any time with 30 days notice to participants.
